To Interested Volunteers:
Thank you for your interest in volunteering at Dr. P. Phillips Hospital. Volunteers are an important part of our organization.
On these web pages you will find the information needed to begin the process of becoming a volunteer. Please print and complete the application packet in its entirety. Be sure to include three references (name, address, phone number and relationship). Try to list at least one business/professional reference. Remember that family members cannot be used as a reference.
You will need to print, review and sign the consent form to test for tuberculosis and drug screening. These screenings will be done at no charge to you. Also, please print and sign the Agreement to Conduct Background Check form. A criminal background check will be performed. Please be aware that we do not accept applications for those individuals attempting to complete court-ordered community service hours.

We will notify you upon receipt of your application. When the reference checks are complete, we will call you to set up an appointment for an interview. Volunteers are also required to attend an orientation session, which will be scheduled after your interview.
